As Care Services Director, you will:

  • Support Program Development and Design:

    • Inform program design based on data evaluation aggregated across multiple sources, including self report assessments, data collected from medical devices, and patient medical records, as well as patient satisfaction surveys and staff feedback.

    • Interact with patients, social services, care/case managers, and provider care teams (nurses, physicians, and other clinical staff) to inform patient interventions and program approach.

    • Work collaboratively with internal and external teams to improve the efficiency and effectiveness of our care services.

    • Work with vendors and internal teams to configure tools and resources for the Care Services team.

    • Build workflows, process and program approach to meet organizational and market needs.

    • Assist VP of Services in development and maintenance of organizational strategy, policies, procedures, workflows and metrics

    • Develop positive relationships with providers and clinical teams within our clinic partnerships maintaining ongoing communication

  • Provide team management and growth

    • As we grow, develop a team of highly skilled staff who work together to achieve Care Services goals. Hire, train, evaluate and oversee staff and staff development process.

    • Provide leadership to staff in a collaborative environment that offers job satisfaction, recognition, and career development.

    • Ensure accountability for high performance in patient satisfaction and the delivery of safe and high quality and scope appropriate patient engagement.

    • Review patient cases with the team and provides advice, direction, and support as needed.

    • Support staff as an escalation and decision point for complex patient needs.

You have the following required experience:

  • Degree in Social work, nursing (BSN preferred), public health, health education, or other health/social science related disciplines required

  • Experience in health care and delivering advocacy, coaching, care coordination or navigation programs required

  • Minimum of 5 years experience leading and directly managing a team of healthcare professionals; experience with leadership in a remote environment required

  • Digital Health experience required

  • Experience in a start-up environment required

  • Experience in program and project management required

  • Experience in Motivational Interviewing required

  • Experience in health equity, social justice and/or trauma informed care required

  • Experience working with diverse populations seeking healthcare preferred but not required

You’re a person who has:

  • Strong ability to lead teams by coordinating, delegating, and supervising staff to perform at their highest level.

  • Desire to interact with patients across social and clinical needs and ability to create strong interpersonal relationships

  • Demonstrated ability to communicate with providers and their care teams to facilitate relationships and high program satisfaction as a trusted resource

  • Passion about driving health equity

  • Strong ability to capture learnings from staff, patient and physician communications and effectively translate them to internal stakeholders and to inform program or product needs

  • Passion about developing and supporting products and programs that improve mental health outcomes

  • Flexibility, creativity, drive and ability to telework in an evolving environment

The ideal candidate is a strong people leader that has an exceptional track record of leading high functioning teams and developing strong relationships with patients and providers while exercising strong judgment in assessing risk and determining escalations to appropriate resources based on a holistic view of the patient and the system in which we operate.

Health Rhythms captures data to create objective and actionable insights to improve mental health outcomes globally. Providers use Health Rhythms data to improve outcomes for their patients, and life science companies use Health Rhythms data to study and create better therapeutic solutions.
